Plan · The care notebook

Keep the open questions visible.

A decision can wait for an answer that matters to you.

Make a note you can return to.

Print this page or copy the prompts into your own notebook. Use “answered”, “need more detail” or “not discussed” beside each topic. The point is to see what remains unclear.

Mark each item with the answer you have. A tick beside a name, a record or a price does not decide whether treatment fits your needs. Take the open questions to the person treating you.

Gather the answers in one place.

My reason for seeking care

What do I want the visit to help me understand?

The assessment

What explanation has the person treating me given, and what information is still needed?

The names and records

Who would treat me, which pharmacy would make the medicine, and which public records did I check?

The evidence

What supports this proposed use, and what does that evidence leave open?

The quote

Which charges are included, which cost extra, and what terms apply if I end the plan?

The review

Who will respond to questions, and how will the plan be reconsidered?

Bring the unresolved part back.

Circle the answer you cannot yet explain in plain words. Ask for that part to be explained again, or take it to another doctor or health professional for a further view.

Keep copies of the proposal, quote and records you used. If the proposal changes, add the change rather than overwriting the earlier note.

Loose blank note sheets and a soft blue bookmark arranged with space between them.
Take unanswered questions about the medicine, price and follow-up back to the person treating you.
